After you have made an appointment, you can review the Notice of Privacy Practices then print and fill out the appropriate forms below and bring them to the our first session.

If this is not convenient, you can complete the forms at the office.

Adults Couples Adolescents (13-17) Children 12 and under
1. After reviewing the Notice of Privacy Practices above, print and sign this: Consent and Acknowledgment Form 1. After each of you have reviewed the Notice of Privacy Practices above, print two copies of this and sign them: Couples Consent and Acknowledgment Form 1. After a parent has reviewed the Notice of Privacy Practices above, the parent should print and sign this: Consent and Acknowledgment Form 1. 2. Then print and fill out these: Intake documents 2. Then, each of you, print and fill out these: Intake documents 2. 3. If anxiety is one of the things you want to work on, print and fill out this as well: GAD assessment 3. If one or both of you has significant anxiety, one or both should print and fill out this as well: GAD assessment 3. A parent should fill out this: Adolescent Parent Questionnaire 3. And these: Parent Questionnaire
4. If you have experienced significant trauma(s) during your life, print and fill out this as well: Trauma Recovery Scale 4. If you either or both have experienced significant trauma(s) during your lives, print and fill out this as well: Trauma Recovery Scale 4. If the adolescent has significant anxiety, the adolescent client should fill this out privately: GAD assessment
5. Both of you should privately fill out this: Partner Questionnaire. Please, paleease do not share your responses with each other. 5. If the adolescent has experienced significant trauma(s) during her or his life, print and have the client privately fill out this as well: Trauma Recovery Scale
